ABSTRACT

Objective:To elucidate the relationship between HLA-DQB1 allele polymorphism as well as the expression level of Th1/Th2 cytokines with familial clustering of hepatocellular carcinoma ( HCC) to provide some evidence for the seeking susceptibility gene or resistant gene of HCC in Guangxi yao,China.Methods:With the same sexuality,age ±5 year,40 members whose families have had two or more HCC patients( high-occurrence families) were selected as the case group,and 40 members whose families have no any cancer patient were selected as the controls.Peripheral blood samples were collected to extract DNA,PCR-SSP was used to detect HLA-DQB1 alleles and ELISA was used to detect IL-2,IL-4 and IL-10.Results:(1) The gene frequency of the HLA-DQB1*02/09 alleles in the case group was higher than that in the controls(P0.05 ).( 2 ) The gene frequency of alleles HLA-DQB1 in HBsAg positive group and HBsAg negative group were never significant difference (P>0.05).(3)The expression levels of IL-4,IL-10 in the case group was higher than that in the control ( P<0.05 ).( 4 ) The expression level of IL-10 in the positive group of the HLA-DQB1*02 allele was higher than that in the negative group of the HLA-DQB1*02 allele ( P<0.05 ).( 5 ) The expression level of IL-4 in the positive group of the HLA-DQB1*09 allele was higher than that in the negative group of the HLA-DQB1*09 allele( P<0.05) .Con-clusion:(1) HLA-DQB1*02/09 seem to be susceptibility genes of hepatocellular carcinoma in high HCC incidence areas of Guangxi yao.(2) There may be not significant correlation bewteen HLA-DQB1 alleles and the susceptibility of HBV infection in high HCC incidence areas of Guangxi yao.( 3 ) The imbalance of IL-4, IL-10 might be associated with familial clustering of hepatocellular carcinoma in Guangxi yao.(4)The imbalance of IL-10 might be due to the carrying of HLA-DQB1*02;the imbalance of IL-4 might be due to the carrying of HLA-DQB1*09.Through the same approaches,these might lead to the phenomenon of familial aggregation of HCC in Guangxiyao.

ABSTRACT

Objective To explore the distribution and characteristics of hepatitis B virus (HBV) genotype in the region in Guangxi with high incidence of primary liver cancer (PLC). Methods 103 pairs of samples from the sex- and age-matched members with HBsAg-positive from PLC-clustering families (the experimental group) and carcinoma-free families (control group) were collected. Nested polymerase chain reaction (PCR) and sequencing methods were applied for the analysis of HBV genotype. Results Four HBV genotypes: B, C, B/C and D, were detected, the percentages of them in the two groups were 31.1%, 63.1%, 1.9%, 1.9% and 30.1%, 55.3%, 6.8%, 2.9%, respectively, showed no significant differences (P > 0.05). HBeAg positive rates were significantly different between genotype C and B (P 0.05). Conclusions The main genotypes were types B and C besides a small number of combined genotypes B/C and D in the regions of Guangxi with a high incidence of PLC. There may be few relationships between HBV genotypes and the high incidence of PLC for familial clustering in Guangxi.

ABSTRACT

ObjectiveTo investigate the effect of plasma exchange with non-bioartificial liver support system on serum manganese levels in patients with hepatic encephalopathy and its clinical significance. MethodsA total of 22 inpatients with hepatic encephalopathy who were admitted to our hospital from October 2007 to July 2011 were treated with plasma exchange, and were divided into improvement group and aggravation group based on prognosis. The serum manganese level was dynamically determined using graphite furnace atomic absorption spectrophotometry, and the changes in manganese levels after treatment and relief or worsening of disease were analyzed. Comparison between the two groups was made using t test. ResultsAfter treatment, the 22 patients all showed significantly reduced manganese levels (22.6±6.9 μg/L vs 36.4±10.6 μg/L, t=4.789, P=0.000). After treatment, 8 cases improved and 14 cases aggravated, and the improvement group showed a significantly lower manganese level than the aggravation group (18.9±6.3 μg/L vs 39.2±9.8 μg/L, t=4.816, P=0000). ConclusionNon-bioartificial liver support system can reduce serum manganese levels in patients with hepatic encephalopathy, and the serum manganese level may influence the disease progression.

ABSTRACT

Objective To explore the relationship between mutations in basic core promoter (BCP) of hepatitis B virus (HBV) and familial clustering of hepatocellular carcinoma (HCC) in Guangxi. Methods 153 pairs of members with HBsAg-positive were selected and matched from HCC high-incidence families and carcinoma-free families in Guangxi. The BCP genes were amplified and sequenced. Results The hotspot sites of the previous five mutations in BCP were T1762, A1764, G1775, V1753, G1803. In univariant analysis, HBV DNA≥105 copies/mL, T1762, A1764 and V1753 mutations were associated with the HCC high-incidence (P <0.05). The multivariate logistic analysis showed that HBV DNA≥105 copies/mL and A1764 were independent risk factors for it. Conclusion HBV DNA level, the mutations in BCP showed correlations with familial clustering of HCC in Guangxi.

ABSTRACT

<p><b>OBJECTIVE</b>To detect hFgl2 expression in peripheral blood mononuclear cells in patients with chronic hepatitis B and liver cancer and explore its association with the severity of chronic hepatitis B.</p><p><b>METHODS</b>The protein expression of hFgl2 in peripheral blood mononuclear cells was detected in 78 patients with chronic hepatitis B (including mild, moderate, or severe cases), chronic severe hepatitis, or liver cancer, with 20 healthy volunteers as controls. The data were analyzed in comparison with the patients' alanine aminotransferase (ALT), aspartate aminotransferase (AST) and total bilirubin (TBiL and) levels.</p><p><b>RESULTS</b>hFgl2 protein expression was significantly higher in patients with chronic severe hepatitis and liver cancer than in the healthy volunteers and patients with chronic hepatitis B. The patients with chronic severe hepatitis had significantly higher hFgl2 protein expression than patients with liver cancer. In severe cases of chronic hepatitis B, hFgl2 protein expression was positively correlated with ALT, AST and TBiL, but these correlations were not found in mild or moderate cases.</p><p><b>CONCLUSIONS</b>Peripheral blood mononuclear cells express hFgl2 protein, whose expression level increases with the severity of chronic hepatitis B.</p>

ABSTRACT

Objective The purpose of this study was to explore the expression of human fibrinogen-like protein 2 (hFgl2) in patients with cryptococcal meningitis (CM).Methods The expression of hFgl2 in the peripheral blood leukocytes from 20 CM cases and 20 healthy individuals,and the expression of hFgl2 in cerebrospinal fluid (CSF) leukocytes in 11 pairs of CM patients and matched controls was detected using immunohistochemical methods.Medical image analysis system (MIAS) was used to analyze the expression of hFgl2 gray values,which were negatively correlated to the expression of hFgl2 protein in both peripheral blood and CSF.Results The average gray values for hFgl2 protein in peripheral monocytes of CM patients and healthy controls were 114.49± 11.82 and 137.56± 10.64,respectively,and those in peripheral neutrophils were 111.71 ± 8.62 and 143.56±12.57,respectively.Compared with the healthy control group,expressions of hFgl2 in peripheral blood monocytes and neutrophils were both significantly increased in CM patients (t =21.21,t=27.19; P<0.01).The average gray values for hFgl2 protein in CSF monocytes from CM patients and negative controls were 115.44±8.67 and 149.25±8.57,respectively,and those in CSFneutrophils were 111.71±8.62 and 143.56 ± 12.57,respectively.The expressions of hFgl2 in CSFmonocytes and neutrophils were significantly increased in CM patients (t =29.95,t=28.87; P<0.01),while the expression intensity of hFgl2 was similar when peripheral blood monocytes and neutrophils compared with their CSF counterparts (t=0.924,t=0.607; both P>0.05).Conclusions hFgl2 can be expressed in both peripheral and CSF monocytes and neutrophils in CM patients.

ABSTRACT

Objective To investigate the effect of Th1/Th2 cytokines and immune state on the occurrence and familial aggregation of hepatocellular carcinoma(HCC).Methods Ninety-five members whose families have had two or even more HCC patients(high-occurrence families) were selected as the case group,by matching with the same nationality,gender,residential area,age±5 years old,95 members whose families had no any cancer were selected as the control.The level of peripheral blood Th1 type cytokines such as interferon-γ(IFN-γ),interleukin-2(IL-2) and Th2 type cytokines such as interleukin-4(IL-4),interleukin-10 (IL-10) were detected by enzyme linked immunosorbent assay(ELISA).Results There was a Th1/Th2 serum cytokine imbalance profile in members of HCC high-occurrence family.The levels of IFN-γ and IL-2 were significantly lower in members of HCC high-occurrence family than that of the controls.The levels of IL-4 and IL-10 were higher in members of HCC high-occurrence family than that of the controls.Conclusion There was a poor cellular immune state in members of HCC in the high-occurrence families.Th1 type cytokines was inhibited,and Th2 type cytokines was enhanced,so more susceptible to HBV chronic infection.It might be the mechanism of HCC occurrence and familial aggregation.

ObjectiveTo elucidate the association of immune response to hepatitis B vaccination with HLA-DRB1 * 12 allele as well as the level of IL-4 and IFN-γ.MethodsSeventy-four healthy college students from Guangxi province who had non- or hypor -response to recombinant hepatitis B vaccination and 64 medium- or hyper-responders with the conditions of similar were selected randomly and involved in this study.HLA-DRB1 * 12 was detected by PCR-SSP,the level of IFN-γ and IL-4 cytokines were examined by ELISA.Results(1)The allelic frequencies of HLA-DRB1 * 12 was lower in the non- or hypor-responders than that in the medium- or hyper-responders ( 10.8% vs 32.8%,P=0.002) ; (2)The expression level of IFN-γ in the non- or hypor-responders ( 7.21±7.92 ) ng/ml was much less than that of the medium- or hyper- responders ( 16.36± 11.00) ng/ml ( P=0.000).(3) The expression level of IL-4 in the non- or hyporresponders (3.18±4.45) ng/ml was much less than that of the medium- or hyper- responders (7.76±5.71 ) ng/ml(P=0.000).(4)No significant differences was seen between the expression level of IFN-γ in the HLA-DRB1 * 12 positive ( 13.18± 11.24) ng/ml and the negative ( 11.00± 10.29 ) ng/ml ( P =0.349 ).(5)No significant differences was seen between the expression level of IL-4 in the HLA-DRB1 * 12 positive (5.947±4.530) ng/ml and the negative (5.132±5.800) ng/ml (P=0.423).ConclusionHLA-DRB1 * 12 might be the allele enhanced immune response to hepatitis B vaccination.The expression levels of IFN-γand IL-4 correlating to Thl/Th2 cells might affect on the immune response to hepatitis B vaccination.

Objective:To investigate the effect of HLA-DRB1*07,13 allelic genes on the immune response to hepatitis B vaccination.Methods:896 healthy college students of Han nationality from Guangxi province,who had received standard courses of vaccination with recombinant hepatitis B virus vaccine,were tested the level of anti-HBs with ELISA method at the sixth month after their last vaccination.The non- or hypo-responders were selected to receive another 20 μg doses of recombinant vaccine and were examined anti-HBs once more four weeks later.99 non- or hypo-responders and 136 medium or hyper-responders were selected for the study subjects.HLA-DRB1*07,13 allelic genes of the subjects were detected by polymerase chain reaction using sequence specific primer (PCR-SSP) method.Results:The frequencies of HLA-DRB1*07 gene in the non- or hypo-responders group were significantly higher than that in the medium or hyper-responder group (16.16% vs 4.41%,P<0.05). No significant difference of HLA-DRB1*13 gene between the two groups (1.01% vs 3.68%,P=0.389).Conclusion:There is a close relationship between the expression of HLA-DRB1*07 gene and the non- or hypo-response to HB vaccine;No relationship between HLA-DRB1*13 gene expression and the response to HB vaccine is found.

ABSTRACT

objective To determine the relationship between immunohistochemical stain assay of liver tissue and prognosis of viral hepatitis.Methods 132 patients with chronic hepatitis B diagnosed 6 to 8 years ago by liver biopsy had been followed up by the retrospective and prospective study.The serum hepatitis B virus markers(HBVm)?alpha-fetoprotein(AFP)?Alanine aminotransferase(ALT) and Aspartate aminotransferase(AST)have been detected and B-scan ultrasound of liver and spleen have performed when they stayed at hospital and during follow-up-time,their symptom and physical sign have been recorded simultaneously.Results 4 of 104(3 85%) patients died of cirrhosis;of 100 survival patients,2 patients(2%,2/100) were typical cirrhosis and 16 (16%,16/100) patients were early stage cirrhosis and parenchyma of liver diffuse lesion diagnosed by ultrasound.The serum ALT of patients whose HBcAg and HBsAg were positive in liver tissue was easier fluctuation than those of negative case.Conclusion The status of patients with HBsAg and HBcAg positive in liver tissue is easier fluctuation than those negative,HBsAg and HBcAg positive in liver tissue maybe have no effect on the prognosis of the chronic hepatitis B.
